Officials Evaluation Request

Complete the officials evaluation request form if you need an evaluation for a particular session. Please submit your request for evaluation before June 19, 2025 noon. Or you can fill in the form at the Official Room BEFORE your working session begins.

Officials can only request ONE (1) sign-off per position per meet.
Officials must have worked at least TWO (2) sessions in a position before requestiong an evaluation and must be able to independently work with the position..
